SACRAMENTO, Calif., April 24, 2025 /PRNewswire/ -- Health Net, a company under Centene Corporation (CNC), has joined forces with Dignity Health and Vivant Health to transform healthcare service delivery for Medi-Cal members in Sacramento County. This innovative collaboration, which places Vivant Health nurses within Dignity Health facilities, has already shown promising results in its first year by avoiding over 400 unnecessary inpatient hospital days and elevating post-discharge follow-up appointment adherence from 76% to an impressive 96%.

The initiative is part of a broader effort to integrate healthcare services with California's CalAIM program, which emphasizes comprehensive care encompassing mental health, substance use interventions, and social determinants of health such as housing and care navigation. Key to this collaboration is the focus on ensuring patients discharged from Mercy San Juan Medical Center and Methodist Hospital of Sacramento receive tailored follow-up care through outpatient services.
